Medicare Part B Premium 2027: Projected Cost, Rebate and Help Paying

The 2027 Medicare Part B premium is projected at $209.50 a month. Here's when the official number comes out, the $90 rebate, and programs that help pay.
The standard Medicare Part B premium is projected to be $209.50 a month in 2027, up $6.60 from $202.90 in 2026. That's a projection from the Medicare Trustees. Medicare will announce the official figure in November.

Be careful with articles that quote $215 or more as the 2027 premium. Those aren't official figures. Projections can also miss: last year's estimate came in about $4 low.
The one-time $90 rebate
About 20.8 million people are getting a one-time $90 payment in October 2026 to help with Part B costs. You qualify if you:
- Have Original Medicare Part B (not Medicare Advantage)
- Live in the U.S.
- Don't get Medicaid help with your premium
- Don't pay the high-income surcharge (IRMAA)
You don't need to sign up. It arrives by direct deposit or Treasury check. Call 1-800-MEDICARE about eligibility, or Social Security about payment status.
Will the premium shrink your Social Security check?
For most people, no. A "hold-harmless" rule says a Part B increase can't make your net Social Security check smaller than the year before. It protects about 70% of people with Part B.
You're not protected if you:
- Pay the high-income surcharge (IRMAA)
- Are new to Medicare or Social Security
- Have Part B paid by Medicaid
- Don't get Social Security checks
Don't sign up late
If you delay Part B without other qualifying coverage, you pay a penalty of 10% of the standard premium for each full 12 months you waited, for as long as you have Part B. If you're covered through a current job, you get eight months after that coverage ends to sign up without a penalty.
Help paying your premium
Medicare Savings Programs pay the Part B premium for people with limited income. 2026 monthly income limits:

The savings limit is $9,950 for a single person and $14,910 for a couple. Qualifying also gets you Extra Help, which caps drug copays. Apply through your state Medicaid office.
What to do now
- Watch for your Social Security COLA notice after Oct. 14. It shows your 2027 premium.
- Check back in November for the official 2027 premium.
- If money is tight, see if you qualify for a Medicare Savings Program.
